if Team A scored a touchdown, and Team B refused to take the field for the PAT, what would be the official's next move?

I’m assuming this is an end of game scenario when the game is well out of hand.

Option 1: Hit the ready for play whistle in an attempt to “call the defense’s bluff.”

Option 2: End or forfeit the game.

If not the end of the game, but say A team is up by 50, and B team is just frustrated. Would the only chose be to declare a forfeit if B team won't take the field?

I don't think there is a set protocol for this situation. It would require a discussion with the coach. Generally speaking, however, a team that refuses to take the field has unilaterally declared the game over. In that case, it's over and we'll let the state sort out the consequences.
