Softball rule question

A batter reaches first base. The coach puts in a pinch runner, who is already in the lineup. The opposing team reports this to umpire. What's the ruling ?

When the player reports in this wouldn't be allowed by the Ump.

The Umpire nor coach knew the rule. The league gurus covered up the whole deal, by denying protest on grounds that it did not affect outcome of game.

If the Ump didn't know the rule, then why are they doing games?

All he would have to do is look at his lineup card when they reported in the game to be a pinch runner.
Player X couldn't be allowed to be a pinch runner, unless they are reentering the game in the same spot they came out of the game earlier.
Even if it didn't affect the game outcome the rule was broken and those a protest should have been allowed.
